Underwear After Surgery: What Actually Helps While You Heal

Say the word "briefs" in a pub and someone will produce the fact. Sperm count. Heat. A study. They will not remember which study, or what it measured, or by how much, but they will remember the direction of travel, and it will be delivered with the confidence of a man reading a headline in 2018 and never returning to the subject.
So let us actually read it, because it is a real study, it deserves better than a pub summary, and its findings are considerably narrower than the reputation they earned.
The paper most people are quoting is from the Harvard T.H. Chan School of Public Health, published in the journal Human Reproduction in 2018. It looked at 656 men who provided 1,186 semen samples between 2000 and 2017. Median age around 35. Slightly over half reported wearing boxers most of the time.
The men in it were all attending a fertility centre with their partners. That is the first thing the headlines dropped and the first thing that matters.
Men who mostly wore boxers had sperm concentration around 25 per cent higher and total sperm count around 17 per cent higher than men who did not. They also had follicle stimulating hormone levels around 14 per cent lower.
That last one is the interesting number, and it is worth understanding. FSH is the hormone that tells the testes to produce more sperm. Higher FSH in the tighter-underwear group suggests the body noticing a shortfall and instructing production to increase, which is a compensation mechanism doing exactly what it exists to do.
The researchers were careful about this, and their own list of limitations is longer than most of the coverage.
It was cross-sectional, meaning it photographed a moment rather than following men over time, so it cannot establish that underwear caused the difference. Underwear type was self-reported on a questionnaire, with all the misclassification that invites. The participants were men at a fertility clinic, so the findings may not generalise to the wider population. And the study had no data on trouser type or on the fabric the underwear was made from, both of which affect scrotal temperature just as much as the cut does.
Then there is the largest gap. The study measured semen parameters, not babies. It did not measure time to pregnancy, and it did not measure live births. Sperm concentration is a useful marker and it is not the same thing as fertility, which is why a difference in concentration between two groups of men, both of whom fall within normal ranges, tells you far less about anyone's chances than the headline implied.
If you and your partner are actively trying to conceive and have reason to be concerned, the heat question is worth taking seriously, and it is worth taking seriously across the board rather than as a single garment decision. Scrotal temperature is affected by laptops, hot baths, saunas, long hours seated, cycling, tight trousers and the ambient heat of the room, and several of those have larger and better documented effects than what you wear underneath. If you are not trying to conceive, the relevance of a 25 per cent difference in sperm concentration to your Tuesday is difficult to overstate as a non-issue. Most men are carrying a supply that is comically surplus to any requirement they will ever have.
And if you are somewhere in between, the practical advice is boring: choose breathable fabric, choose the right size, and do not wear anything so tight that it is uncomfortable, which is advice about comfort that happens to also be advice about heat. One observational study of men at a fertility clinic found a moderate difference in semen markers, alongside a hormone finding suggesting the body compensating, and no measurement of actual fertility outcomes at all. That is the whole thing. It is a legitimate piece of research and a completely inadequate reason for a healthy man to choose worse-fitting underwear for the next forty years.

The evidence does not support a clear answer, and the one widely quoted study is much weaker than the headlines it produced. It found a moderate difference in semen markers among men attending a fertility clinic, it did not measure whether anyone went on to father a child, and it could not rule out everything else that differed between the two groups. Markers are not outcomes.
There is no good evidence that changing the cut of your underwear changes your chances, and a doctor is a better first call than a drawer. If heat is a genuine concern, the things with actual evidence behind them are hot tubs, saunas, laptops on laps and long periods of sitting, none of which are solved by loose boxers. Sustained heat to the area does appear to matter, which is why the theory sounded plausible in the first place. The gap in the argument is that ordinary underwear, worn under ordinary clothes, does not produce anything like the temperature rise of a sauna or a fever. The mechanism is real and the everyday application of it is very weak.
